By accessing or using services provided by Confirmed Medical LLC (“Confirmed,” “we,” “our,” or “us”), you agree to be bound by the following terms and conditions. Please read them carefully.

1. Services

Confirmed provides concierge-level medical management services to professional athletes and high-performance individuals. This includes, but is not limited to, injury evaluation, coordination of care with third-party specialists, performance consultations, and health monitoring. All services are subject to availability and scope outlined in individual agreements.

2. No Guarantee of Outcomes

While we provide access to top-tier providers and resources, Confirmed does not guarantee specific medical or performance outcomes. Clinical care decisions remain the responsibility of licensed treating providers.

3. Client Responsibilities

Clients must provide accurate, up-to-date health information and notify Confirmed of any changes relevant to their care. Clients are responsible for all third-party medical costs outside the scope of Confirmed’s direct services.

4. Confidentiality

Confirmed will protect the confidentiality of your information in accordance with applicable federal and state laws, including HIPAA. We do not disclose any client information without explicit written consent unless required by law.

5. Payment Terms

Fees are outlined in your individual service agreement. Payment is due as specified in the agreement. Late or missed payments may result in the suspension of services.

6. Limitation of Liability

Confirmed is not liable for any direct or indirect losses, injuries, or damages arising from third-party provider actions or client failure to follow medical advice.

7. Termination

Either party may terminate the service relationship with written notice. Upon termination, Confirmed will assist in transitioning care but is not responsible for continued management.
